To drive legally in the State of Nevada, a resident must have a valid driver’s license.
Form DMV 002, Application for Driving Privileges or ID Card, or the Nevada Driver’s License Application form is a legal, state-specific form Nevada residents use to apply for a driver’s license. Aside from its use as an application form, it is also used to apply for a renewal or replacement of Nevada licenses and permits.
If you are not a resident of the State of Nevada, you cannot use the Nevada Driver’s License Application form.
The Nevada Driver’s License Application form is a three-page document. Answer all the required information correctly to avoid any problems with your application. Do not deliberately enter any false information as it is punishable by the law.
Application Information
Mark the box to determine your application. You can select Original, Renewal, Duplicate, or Instruction Permit.
License or Permit
Mark the box to determine the type of license or permit you are applying for. You can select Real ID, Driver Authorization Card, or Standard.
Classification

Mark the box to determine your endorsements. You can select J, F, or G.

Mark the appropriate box if you would like to change information on your existing card. You can select Name, Address, Date of Birth, Social Security Number, or Sex.

Enter your full legal name in the following format: Last Name, First Name, Middle Name, Suffix.
Nevada DL/DAC/ID Number
Enter your Nevada driver’s license, driving authorization card, or identification number.
Social Security Number
Enter your nine-digit social security number.
Date of Birth
Enter your date of birth.
Full Legal Name on Birth Certificate
Enter the name that appears on your birth certificate.
Birthplace
Enter your place of birth, including city and state or country.
Sex
Circle M if male and F if female.
Height
Enter your height in feet and inches.
Weight
Enter your weight in pounds.
Hair Color
Enter the natural color of your hair.
Eye Color
Enter the natural color of your eye.
Mother’s Maiden Name
Enter your mother’s maiden name.
Do not scan my birth certificate
Mark the box if you do not want to scan your birth certificate.
Beside this item, mark the box to place your mailing address on the front of your card.
Primary Physical Address
Enter your full primary address, including city, state, and ZIP code.
Mailing Address
Enter your full mailing address, including city, state, and ZIP code, if it is different from your physical address.
Daytime Phone Number
Enter your daytime phone number.
Email Address
Enter your email address.
Other Information
In this section of the form, you will be asked to provide a series of answers. Denote your answer by marking the appropriate box and writing the required information when asked of you.

Affidavit
In this section of the form, you will be asked to verify all information previously placed.
If you are a minor, you must obtain your parent’s written consent. They have to write their name in the appropriate space.
If you would like to be an organ donor, your parents will also have to be informed of that.
You will also have to note that you have not operated a vehicle since the last date of operating a vehicle.
To certify all the information in this form up to this point, you will have to place your signature, your parent’s signature if you are under the age of 18, the date of signature, and the DL/DAC/ID number.
Voter Registration
You can also use this form to update your voter registration information.
You first need to clarify whether you are a US citizen and if you will be 18 years of age by Election Day.
You will also have to provide your phone number and email address but these are not required for the purpose of completing this form.

Use the File button to access functions such as New PDF, New Page, Organize Pages, Save, Save As, Print, Export, Share Link, Email, and Postal Mail.
Use the Text button to add texts, numbers, and symbols to your document. Initially, it will display the current date, but you will be able to change it. To customize the date, you will be able to access additional tools, such as the Move text button, the Font Styles and the Font Sizes drop-down lists, the Bold, Italics, and Underline buttons, and the text alignment options, such as Left, Center, Right, and Justified buttons.
Use the Check button to add a check symbol to your document. To customize the check symbol, you will be able to access the Size adjustment tool.
Use the Cross button to add a cross symbol to your document. To customize the cross symbol, you will be able to access the Size adjustment tool.
Use the Whiteout button to white out or erase items in your document.
Use the Redact button to censor items in your document.
Use the Marker button to highlight items in your document. To customize the marker, you will be able to access the Color selection and Size adjustment tools.
Use the Insert button to add Icons, Header, Footer, Watermark, Comment, and Sticky Note to your document. The Icons button allows you select shapes such as Arrow Left, Arrow Right, Arrow Up, Arrow Down, Thumbs Up Thumbs Down, Question Mark, Exclamation Point, Circle, and Square; the Header button allows you to add a header; the Footer button allows you to add a footer; the Watermark button allows you to add a watermark by typing or uploading an image; the Comment button allows you to leave a comment in any part of the document; the Sticky Note allows you to post a note to any part of the document.
Use the Zoom button to zoom in and zoom out your document.
Use the Undo button to negate the last command done to your document.
Use the Redo button to reverse your last Undo.
