The Nevada Driver’s License Replacement form, otherwise known as DMV 002, is a form issued by the Nevada Department of Motor Vehicles. This form is filled out and used to apply for the issuance of a new, renewed, or replacement license of all Nevada licenses and permits.
If you are a citizen of Nevada and would like to apply for a new, renewed, or replacement driver’s license or permit, then you are currently eligible to fill out the Nevada Driver’s License Replacement form.
To fill out the Nevada Driver’s License Replacement form, you will need to provide your personal information and answer a few questions.
You will have to know the type and classification of the license or permit you are applying for. You have to know whether you are applying for a(n): original license, renewal, duplicate or replacement, or instruction permit.
If you are applying for a license or a permit, you will have to know what kind you are applying for. You can choose among the following: Real ID, Standard, Driving Authorization Card.
If you are applying for an identification card, you must choose from the following: Real ID, Standard, Seasonal Resident.
You will also need to know the classification of the license. The different classifications you may choose from are Class C, Class A, Class M, and Class B.
You will also need to indicate your endorsements. You may choose from the following: J, G, and F.
If you would like to change information on your existing card, then you may choose which information you would like to update among the choices of: name, address, date of birth, social security number, and sex.
Afterward, you will be asked to indicate your full legal name which is composed of your last name, first name, middle name, and suffix. If applying for a replacement, you’ll be asked to write your driver’s license, driver authorization card, or identification card number. You also need your mother’s maiden name.
You will also have to provide your social security number (SSN), however this is optional for those applying for a driver authorization card. You will also have to provide your date of birth, full birth name, and birthplace.
Additionally, you will need to provide information regarding your physical appearance. Namely, you need to write down your sex, height, weight, hair color, and eye color.
You will also be needing the city, state, and ZIP code of your residential and mailing addresses.
You may opt to indicate your daytime phone number, which is the number through which you can be reached between the hours of 9:00 AM and 5:00 PM, and your email address but these are not required for the purpose of filling out this form.
You will also be asked a series of “Yes or No” questions. Specifically, you will be asked if:

Additionally, you will have to sign a waiver signifying that all the information placed within the form is truthful and accurate. You will have to place your signature, parent’s signature if you are under the age of 18, date of signature, and DL/DAC/ID number.
