Form DS-2029, Application for Consular Report of Birth Abroad of a Citizen of the United States of America (CRBA), by the United States Embassy is a form that allows a United States (U.S) citizen to apply to have his or her child become a U.S Citizen. It is necessary to apply for CRBA because the child abroad does not automatically become a U.S citizen even with both of his/her parents being one. However, one should take note that the form is only applicable to children under the age of 18. The eligibility for CRBA issuance depends on several factors which include the following:
In general, the application for CRBA may be made in person at the Embassy after scheduling an appointment on the Embassy’s website, or by telephone. The Embassy might require some specific documentation. Thus, make sure to prepare the following original documents before your scheduled appointment date:
Form DS 2029, Application for Consular Report of Birth Abroad of a Citizen of the United States of America (CRBA), by the United States Embassy, is a 6-page document that needs to be filled out correctly by a US citizen parent to apply to have his or her child to become a citizen of the United States. Below is the information needed for one to fill out the form correctly.
Item 1: Name of the Child in Full
Enter the name of the child that appears on the birth certificate. If the birth certificate shows a different name, the parent or legal guardian must present an explanatory affidavit regarding the child’s correct name. When the name of the child has been legally changed, by adoption or other legal action that amends the name of the child from birth, the parent or legal guardian should support it with documentary evidence. It is only then that the new name may be recorded.

Mark the appropriate box to indicate the gender of the child. You may select:

Enter the date of birth of the child with the month first, then followed by the date and year.
Item 4: Place of Birth:
Enter the place of birth of the child including the name of the city and the country.
Items 5 and 11: Full Name
Enter the full name of the mother, father, or parent as it appears on the passport or any government-issued identity document.
Items 6 and 12: All Previous Legal Names
Enter all legal names ever used by the Mother/Father/Parent including the name given at birth.
Items 7 and 13: Sex
Mark the appropriate box to indicate your gender. You may select:
Items 8 and 14: Date of Birth
Enter the date of birth of the mother, father, or parent with the month first followed by the date and year.
Items 9 and 15: Place of Birth
Enter the place of birth of the mother, father, or parent. Enter the name of the city, state or province, and county).
Items 10 and 16: Current Physical Address
Enter the current physical address of the mother, father, or parent. Do not list a P.O Box. If the application is completed in a foreign country, you may Enter an A.P.O Address.
Item 17: Mailing Address
Enter your mailing address.
Items 18 and 19: Citizenship
Mark the appropriate box to indicate whether the mother, father, or parent is a US Citizen, or Non-Citizen National when the child was born.
Item 20: Marriage to the Biological Parent:
Mark "Yes" if you and a U.S citizen parent of the child were married to the biological parent of the child when the child was born; otherwise, mark "No".
Item 21: Date and Place of Marriage
Enter the date and place of marriage. Mark the current status of that marriage. You may select:
If applicable, Enter the date of divorce or death.
Items 22 and 23: Marriage/s
Enter a list of any other marriages following the format: Date of marriage, end date, manner it ended (if applicable). If you have never been married, write “NONE” in this section.
Items 24 and 25: Time Spent in the United States
Enter a list of dates you have been present in the United States.
Items 26 and 27: Time Spent Abroad in U.S Armed Forces, in other Government employment, with qualifying international organization, or as a dependent child of a person so employed

This part of Form DS-2029 should be completed before or by a consular officer, notary public, or other persons qualified to administer the oath.

Enter the name of the persons providing information, the relationship to the child, and affix your signature.
Item 30: Approval of Consular Report of Birth
The United States Embassy or consulate official approving the issuance of the Consular Report of Birth Abroad will enter the serial number of the Consular Report of Birth Abroad and the date and place of issuance. Then, the official will sign this section.

Mark the appropriate box to indicate the documents presented by the mother, father, or parent. Provide the necessary details asked.

Use the Check button to add a check symbol to your document. To customize the check symbol, you will be able to access the Size adjustment tool.
Use the Cross button to add a cross symbol to your document. To customize the cross symbol, you will be able to access the Size adjustment tool.
Use the Whiteout button to white out or erase items in your document.
Use the Redact button to censor items in your document.
Use the Marker button to highlight items in your document. To customize the marker, you will be able to access the Color selection and Size adjustment tools.
